A Journal Log Assessment Element allows you to create a Clinical Note for a patient from structured inputs.
Initial setup
Create an assessment.
Add the following assessment elements:
- Patient ID
- Rich Text Area named "comments_about_injury"
- Select named "injury_source"
- Checkbox named "affects_soft_tissue"
- Journal Log named "injury_clinical_note"
Template for the Note
The Journal Log supports HTML markup; suitable to place in a Clinical Note.
Simply include the field name from your input in braces; as below:
Be sure to run the Check process to discover any issues.
When ready, publish & enable as per a normal assessment

This results in the following clinical note:
