SA Cancer Reporting Form

To print the SA Cancer Reporting Form:

1. Search for a patient.
2. Click Show.
3. Click Print Sacr Form.
4. The file will be saved to your local or network drive as a PDF file, at which point it can be printed.

This will print the form with the left hand side of the page pre-populated (the right hand side will be blank as this is data relating to an admission). You can also print this form from within an admission and available fields from the admission will be pre-populated.
